1.  fill out form first 
2.  Make sure your curser is outside of information boxes.
3.  Click Edit on your browser bar, a drop down menu will appear.
4.  Click on Select All,  the whole page will be highlighted.
5.  Click Edit on your browser bar again.  Click Copy, the entire form will be copied.  
6.  Click Contact us to bring up email.
7.  Move curser to the body of the email by clicking  in the body of the email.
8.  Right click your mouse.  Click Paste,  this form will be pasted in the body of the email.
